The cell proliferation rate can be assessed by Kiimmunohistochemical IHC staining, and this can be correlated to the tumor grade and the clinical course. This protocol can be used to assess tumors by determining the Ki positivity. The protocol detects and classifies nuclei as positive or negative and returns an average proliferation index for the entire tumor region.

The Ki labeling index LI is an important prognostic factor in breast carcinoma. The Ki LI is traditionally calculated via unaided microscopic estimation; however, inter-observer and intra-observer variability and low reproducibility are problems with this visual assessment VA method. For more accurate assessment and better reproducibility with Ki LI, digital image analysis was introduced recently.
At our institution, this imaging modality is used for the further analysis of inconclusive findings on conventional imaging, for the evaluation of response to different therapies such as neoadjuvant chemotherapy or breast conservative therapy and in yearly screening of women at increased risk for breast cancer, among others. At our institution, breast MRI has already been used for over 10 years for the early detection of breast cancer in intermediate and high risk women. The use of breast MRI as a screening modality on women at elevated risk has been only evaluated on prospective trials but its performance on real clinical practice remains unknown. We, therefore, are investigating the performance of the surveillance program for intermediate and high risk women carried out at our institution.
